I do like the idea of losing limbs on a crit, but it seems too harsh. I mean some freaking kobold chops your 6th level fighters arm off? and he can't use a shield anymore. I think if you want to make a dungeon or scenario more difficult try stepping up the traps or make the enemies harder to avoid, but simply making it more deadly by making a diceroll capable of incapacitating your fighter then it really loses part of its authenticity. I mean what is the point of developing a character if he is just gonna get his leg lopped off? not a lot of adventuring gimps. The aspects i do like about it is that it makes the characters a lot more scared to go into combat for the fear of losing a limb or something, but i think occassionaly tossing in a real powerful creature will also keep that fear strong in them and then you don't have to retire so many characters.

I think vorpal weapons cover the hacking of limbs off well enough, no need to impliment them in normal combat, other wise whats the point?
